Abstract:Objectice: Traditional Chinese medicine has a long history of over 2,000 years in treating piles, based on the principle “Sore herbs can be stringent while the puckery can control prolapse”, and a large amount of precious experience has been accumulated. Methods: Prepared mainly with such effective compositions as gallnut (Galla Chinensis) and alum, Xiaozhiling (XZL) was applied in treating piles of stages III and IV. Fourstep injections were given at (1) the branches of artery rectalis superior (ARS), (2) the submucosa, (3) the mucous lamina propria, and (4) the sinous veins. The injection obliterated the ARS and made piles sclerosed, atrophied and finally disappear. Results: From 1987 to 1996, piles of stages III and IV were treated of 21,361 cases. Of all the cases, 21,148 (99%) were cured, 203 (0.95%) improved and 10 (0.05%) ineffective for whom surgery was used. Of the 620 cases who were followed up 3 years after treatment, it recurred in only 6 cases (prolapse, 1 %).Conclusion: This method summarized a relatively ideal non-operative approach in the treatment of piles of stages III and IV.

Abstract:Objective: To observe the effect of acute lung injury after abdominal surgical operations treated with herbs activating blood circulation and dispersing stasis. Methods: Forty-four patients complication with acute lung injury after abdominal operation were analyze with multiple clinical and laboratory parameters and were treated early with herbs activating blood circulation and dispersing stasis. Results: After 2 days of medication, arterial oxygen saturation (SaO2) and partial pressure of oxygen in artery (PaO2) markedly improved, with significant difference as compared with those before medication (P<0.01). After continous medication for 4 days, SaO2, PaO2 and chest X-ray as well as synthetic physiologic parameters obviously improved, prostaglandin F1a, thromboxane B2 (TXB2), C-reactive protein (CRP), erythrocyte C3b were very significantly different from those medication (P<0.01), the synthetic physiological parameters returned generally to normal. After 5 days of herbal medication, all parameters resumed normal with satisfactory physiologic criteria. Conclusions: Early application of herbs activating blood circulation and dispersing stasis could obviously improve hypoxemia and the lung lesions, and consequently the general condition of the patient.

Abstract:Objective: To study the pathogenetic mechanism of integrated traditional Chinese and Western medicine in treating lower limb deep venous thrombosis (DVT).Methods: Thrombosis test in vitro of 206 cases with lower limb DVT was measured. All of them were treated by integrated traditional Chinese and Western medicine. Results: The indexes of thrombosis test in vitro of patients were much higher than those of normal subjects (P < 0.001). The relevent indexes were different in type distribution according to differentiation of symptoms and signs (P < 0.01~0.001) and between pre-treatment and post-treatment (P < 0.001). It showed that hypercoagulability of blood occurred even after DVT took place. Changes of indexes, when after treatment they got lower along with improvement in the state of illness, might be objective parameters for Syndrome Differentiation in traditional Chinese medicine. Conclusions: The therapy of traditional Chinese and Western medicine has good efficacy for DVT.

Abstract:Objective: The effect and side-effect of epidural injection with lappaconitine compound for post-operative analgesia was observed. Methods: One hundred and twenty patients were randomly divided into 4 groups. Lappaconitine compound (LB) consisted of 12 mg of lappaconitine and 22.5 mg of bupivacaine, was used for the observed group, and lappaconitine 12 mg, bupivacaine 22.5 mg and morphine 2 mg for the control group respectively, All were given by epidural injection with single blind method during post-operative pain of incision operation. Results: It was showed that the initiating of analgesia was earlier in LB and bupivacaine group than that in group B and D, and the efficacy was morphine group > LB > bupivacaine > lappaconitine. There was significant difference between group A and B in the above two parameters,P < 0.01 andP < 0.05. The analgesia maintenance time of a single injection was morphine >LB> lappaconitine> bupivacaine, that of group D was significantly longer than that of group A (P < 0.01).Conclusions: It indicated that the epidural injection with LB was more rapid and potent than that with lappaconitine alone in post-operative analgesia. Besides, it had no side-effect and safer than morphine.

Abstract:Objective: To observe the effects of berberine on insulin sensitivity in high-fat-fed rats. Methods: Before and after ingesting berberine, glucose insulin tolerance test was used to measure the insulin sensitivity, and to determine the fasting blood glucose, insulin, blood lipid and muscle triglyceride content. Results: Compared with normally fed rats, high-fat-fed rats had their insulin sensitivity lowered (K value 5.02 ± 1.08 vs 8.72+0.91,P <0.005), level of liver glycogen decreased and levels of fasting blood glucose, insulin, lipid and muscle triglyceride content raised. Berberine and metformin improved insulin resistance (K value 6.31 ± 0. 95 and 6.24 ± 0.65) and liver glycogen level in insulin resistant models, though they had no effect on blood glucose, insulin, lipid levels and muscle triglyceride depots. Conclusion: It is confirmed that berberine had similar effect in raising insulin sensitivity of high-fat-fed rats.

Abstract:Objective: To investigate the influence of traditional Chinese medicine Qingjieling on leukocyte adhesion to vascular endothelial cells and the possible mechanisms. Methods: Firstly, We successfully established in vitro culture of human umbilical vein endothelial cells (HUVEC) monolayer. Then we constructed a leukocyte-endothelial cell adhesion model and imitated the inflammation state by using endothelial cells stimulated with recombinant tumor necrosis factor a (TNFα) or interleukin-1 (IL-1) respectively. The drug-induced change of leukocyte adhesion function was observed after we directly administered traditional Chinese medicine Qingjieling on the leukocytes. Finally, we inquired into the molecular mechanism of the effect of Qingjieling by monoclonal antibody blocking assay. Results: HUVEC as a representative large vessel endothelial cell could perfectly construct an adhesion model with leukocytes. Under the stimulation of cytokines (TNFα or IL-1), endothelial cells demonstrated drastically improved adhesion with leukocytes, certain dose of Qingjieling (1mg/g body weight) might augment the leukocyte adhesion capacity with a 2-hour co-incubation. At least part of the reason of Qingjieling’s improving adhesion effect is due to the increased lymphocyte functional associated antigen-1 (LFA-1) expression of the white blood cells. Conclusion: Qingjieling could increase LFA-1-dependent leukocytes adhesion to endothelial cells.

Abstract:Objective: Research and development for the present subject is oriented to the identification of ”new vision of Integrated Medicine” for health promotion and disease control from the viewpoint of holistic approach. Methods: The ”holistic approach” in the present study comes from ”general network approach” for unity in diversity which includes ideas, theories and methods on health culture and scientific technology. ”Essential quality” is the spirit of systems regulation for common problem solving focusing on cultural norm from the viewpoint of the recovery of humanity. Results: The new vision of Integrated Medicine is Two-in-One nature of oriental and western medicine through the unification of community medicine and community control with trinity approach on education, practice and research. Conclusions: We have tried to reevaluate the existent idea of integrated medicine through an educational seminar in Hebei, China in 1996. Based on this experience, we have identified the global vision of Integrated Medicine which includes new vision of Integrated Medicine and conventional sense of Integrated Medicine in the present study.
