      Abstract:To investigate the relationship between the severity of Chinese medicine (CM) bloodstasis syndrome (BSS) with clinical features and renal lesion indexes of the primary glomerular disease. An epidemiological survey was conducted to collect the data of 227 patients diagnosed as chronic primary glomerular diseases, and their severity of BSS were scored three days before renal biopsies were performed. The following clinical indexes were analyzed: age, course of glomerular diseases, 24-h urine protein ration (Upro), hypertension and blood pressure (BP) progress, serum creatinine levels (Scr), estimation of glomerular filtration rate based on the predigesting equation of MDRD (eGFR), blood urea nitrogen (BUN), uric acid (UA), triglyceride (TG), cholesterol (CHO), haematoglobin (HGB), albumin (ALB), and the correlation among renal pathological types, pathology lesion indexes, and BSS scores. (1) Among the 227 patients, 207 (91.19%) were diagnosed as BSS, in which 95 cases were considered as moderate and the rest 112 cases as severe. (2) There was a negative correlation between age, gender, grades of the hypertension, and the BSS score. Multiple stepwise regression analysis showed that Upro, CHO, TG, and eGFR were positively related to the BSS score (P<0.05). (3) The BSS score has a positive correlation with indexes of chronic renal pathology, especially the tubular atrophy and interstitial fibrosis. The severity of proliferation and glomerular sclerosis was accompanied with higher BSS scores with a significant difference (P<0.05). BSS is one of the most common CM syndromes among patients with the primary glomerular diseases; the BSS score has a positive correlation with Upro, CHO, TG, eGFR, as well as the index of chronic renal pathology. Based on these observations, the BSS may be used as an indicator of the development of renal diseases. Being positively diagnosed as BSS could indicate the beginning of the chronic phase of the primary glomerular diseases.

      Abstract:To determine whether the ingestion of a herbal supplement called Rhodiola-Gingko Capsule (RGC) would enhance the endurance performance of healthy volunteers and change relevant hormones in a favorable manner. Seventy healthy male volunteers (age ranges from 18 to 22 years old) were randomly assigned to RGC group (35 cases, each capsule containing 270 mg herbal extracts, 4 capsules per day) or placebo group (35 cases, equivalent placebo preparation) for 7 weeks using computerproduced digital random method. The endurance performance, serum testosterone and cortisol levels were measured at the baseline and the endpoint. Sixty-seven subjects (34 in the RGC group and 33 in the placebo group) completed a 7-week treatment. The RGC group displayed a significantly greater baseline-toendpoint increase in maximal oxygen uptake (VO2max) than placebo group in both absolute (P=0.020) and relative values (P=0.023). At the endpoint, the serum cortisol level was unchanged in the RGC group compared with the baseline, but it was significantly elevated in the placebo group (P<0.05). The endpoint ratio of testosterone to cortisol, a surrogate for overtraining and fatigue in endurance exercises, was also indifferent compared with the baseline in the RGC group, but significantly decreased in the placebo group (P<0.05). The combined herbal supplement of Rhodiola and Gingko could improve the endurance performance by increasing oxygen consumption and protecting against fatigue.

      Abstract:To observe the therapeutic efficacy of Danggui Buxue Decoction No.1 (当归补血汤1号, DB1) in treating patients of non-small cell lung cancer (NSCLC) at the peri-operational stage and its impact on the patients’ immune function. Eighty-two NSCLC patients were randomly assigned to two groups equally, the control group and the test group, they were given conventional treatment, while to the test group, DB1 were given additionally. The observation was conducted by testing the changes of T-lymphocyte subsets, natural killer (NK) cell activity, serum levels of immunoglobulin (IgA, IgM, IgG), interleukin-2 (IL-2), tumor necrosis factor α (TNF-α), cytokeratin fragment 19 (CYFRA21-1) and carcinoembryonic antigen (CEA) in NSCLC patients before and after administration of DB1, and analyzing the patients’ general condition. The level of CD3+, CD4+, the ratio of CD4 and CD8+, IgA, IgM, IgG and IL-2 decreased in patients with NSCLC on day 1 after operation, and the level of CD8 and TNF-α increased compared to pre-operation. While the levels of CD3+, CD4+, CD4 /CD8+, NK cell activity, serum IgA, IgM, IgG, IL-2 began to elevate, CD8 and TNF-α levels began to decline in patients administered with DB1 on day 3 after the operation, earlier than patients who did not use the decoction. The level of CYFRA21-1 and CEA, was immediately decreased after operation in both groups. Applying DB1 to NSCLC patients at an early post-operational stage could alleviate the impairment and accelerate the recovery of immune function of patients to enhance their immunity. DB1 also shows an anti-tumor action to a certain degree.

      Abstract:The study aimed to evaluate and compare the efficacy and safety of dihydroartemisinin-piperaquine phosphate (Artekin) and artemisinin-piperaquine (Artequick) in the treatment of uncomplicated falciparum malaria. A total of 103 uncomplicated falciparum malaria patients were enrolled and randomly assigned to two groups: 52 cases in the Artequick group, and 51 cases in the Artekin group. The patients in the Artequick group were administered with Artequick, twice in 24 h, whereas the patients in the Artekin group were given Artekin 4 times in 2 days. The mean parasite clearance time, mean fever clearance time, 28-day cure rate and parasite recrudescence rates of the two groups were then compared. The mean parasite clearance time and the mean fever clearance time were 43.2±13.9 h and 24.7±9.9 h, in the Artequick group, and 36.5±17.1 h and 22.7±11.2 h, in the Artekin group. In both groups the 28-day cure rate was 100%, and the parasite recrudescence rate was 0. Both medicines had high cure rates, low recrudescence rates, and no serious adverse reactions. The administration of Artequick, however, was more convenient and lower incidence of gastrointestinal side effects than that of Artekin, so as to increase the efficacy in the malaria population.

      Abstract:To observe the clinical efficacy of Chinese drugs combined with chemotherapy in the treatment of acute myeloid leukemia (AML) and to investigate the prognostic relevance of the main parameters in AML treated with integrative medicine. Forty AML patients hospitalized at the authors hospital were treated with Chinese drugs and chemotherapy. The routine examination, immunophenotype and karyotype analyses were carried out. The clinical efficacy was observed and the prognostic factors were analyzed. (1) Clinical efficacy: Twenty patients had complete remission (CR), with the CR rate being 50.0%. Among these patients, the CR rate was 73.9% (17/23) in de novo AML and 17.6% (3/17) in secondary or refractory AML, respectively. The median disease free survival (DFS) was 6 months (2–32 months) and median overall survival (OS) was 7 months (1–36 months). (2) Analysis of prognostic factors: Aging (> 60 years) and hepatosplenomegaly or extramedullary leukemia did not affect the treatment outcome. Patients with lower white blood cell (WBC) counts (<4.0×109/L) had a significantly higher CR rate (P<0.01). Secondary or refractory AML was associated with a lower CR rate and shorter OS (P<0.01,P<0.05). Expression of CD34 was an adverse factor for obtaining CR (P<0.05) and survival in both DFS and OS (P<0.05,P<0.01). The expression of CD56 was significantly associated with a lower CR rate (P<0.05), but did not affect DFS and OS. Twenty-three (57.5%) out of 40 cases had chromosomal abnormalities. The CR rate was decreased and both DFS and OS shortened stepwise from the cases with favorable cytogenetics to those with intermediate and unfavorable cytogenetics (P<0.01). The combined treatment of Chinese drugs with chemotherapy has a predominant effect in de novo AML. Secondary or refractory AML, expression of CD34 and CD56, and unfavorable cytogenetics were the main factors of poor prognosis in AML.

      Abstract:To evaluate the effect of Jianpi Huoxue decoction (健脾活血方, JHD)-containing serum on tumor necrosis factor-α (TNF-α) secretion and endotoxin receptor gene expression in RAW264.7 cells induced by lipopolysaccharide (LPS). The cytotoxicity of blank-control serum and JHD-containing serum at different concentrations were evaluated through the lactate dehydrogenase (LDH) assay in RAW264.7 cells. RAW264.7 cells were divided into six groups: 5% blank-control serum group (C1, n=3), 5% blank-control serum plus LPS group (L1, n=4), 5% JHD-containing serum plus LPS group (J1, n=4), 10% blank-control serum group (C2, n=3), 10% blank-control serum plus LPS group (L2, n=4), and 10% JHD-containing serum plus LPS group (J2, n=4). After cultured with the corresponding serum for 1 h, cells in L1, L2, J1 and J2 were treated with LPS (0.1 μ g/mL) for 12 h without rinse. The supernate, cells, protein and RNA were collected for assay. TNF-α in the culture supernate was assayed by the enzyme linked immunosorbent assay (ELISA). Protein expression of TNF-α in RAW cells was detected by Western-blot. TNF-α, Toll-like receptor 2 (TLR2), TLR4 and CD14 mRNA expression in RAW cells were detected by real-time RT-PCR. The LDH assay supported that cultured for 24 h or less with the JHD-containing serum at the concentration of 10% or lower, RAW264.7 cells showed no cytotoxicity. After stimulation with LPS for 2 h, TNF-α in the culture supernate of the 5% blank-control serum plus LPS group (L1, P=0.03), 10% blank-control serum plus LPS group (L2, P=0.002) and in the cell layer (P=0.01) of these groups increased remarkably. After stimulation with LPS for 1 h, the mRNA expression of TNF-α (P=0.004), TLR (P=0.03), CD14 (P=0.004) was up-regulated obviously. In the 10% JHD-containing serum plus LPS group (J2), the protein expression of TNF-α in both supernate (P=0.04) and cell layer (P=0.04), gene expression of TNF-α (P=0.03), TLR4 (P=0.001), CD14 (P=0.001) were all inhibited. On the other hand, the TLR2 mRNA expression was not up-regulated after LPS stimulation in the 10% blank-control serum plus LPS group (L2). JHD-containing serum inhibited the LPS-induced cytokines expression in RAW264.7 which was probably associated with its inhibitory effect on the mRNA expression of LPS receptors TLR and CD14.

      Abstract:To optimize the animal model of liver injury that can properly represent the pathological characteristics of dampness-heat jaundice syndrome of traditional Chinese medicine. The liver injury in the model rat was induced by α-naphthylisothiocyanate (ANIT) and carbon tetrachloride (CCl4 ) respectively, and the effects of Yinchenhao Decoction (茵陈蒿汤, YCHD), a proved effective Chinese medical formula for treating the dampness-heat jaundice syndrome in clinic, on the two liver injury models were evaluated by analyzing the serum level of alanine aminotransferase (ALT), asparate aminotransferase (AST), alkaline phosphatase (ALP), malondialchehyche (MDA), total bilirubin (T-BIL), superoxide dismutase (SOD), glutathione peroxidase (GSH-PX) as well as the ratio of liver weight to body weight. The experimental data were analyzed by principal component analytical method of pattern recognition. The ratio of liver weight to body weight was significantly elevated in the ANIT and CCl4 groups when compared with that in the normal control (P<0.01). The contents of ALT and T-BIL were significantly higher in the ANIT group than in the normal control (P<0.05,P<0.01), and the levels of AST, ALT and ALP were significantly elevated in CCl4 group relative to those in the normal control P<0.01). In the YCHD group, the increase in AST, ALT and ALP levels was significantly reduced (P<0.05, P<0.01), but with no significant increase in serum T-BIL. In the CCl4 intoxicated group, the MDA content was significantly increased and SOD, GSH-PX activities decreased significantly compared with those in the normal control group, respectively (P<0.01). The increase in MDA induced by CCl4 was significantly reduced by YCHD P<0.05). YCHD showed significant effects on preventing liver injury progression induced by CCl4, and the closest or most suitable animal model for damp-heat jaundice syndrome may be the one induced by CCl4.

      Abstract:To study the anti-inflammatory mechanisms of total glycosides of Acanthopanax Giraldii (TGA). The changes of prostaglandin E2(PGE2), tumor necrosis factor (TNF-α), nitric oxide (NO), and expressions of COX-1 mRNA and COX-2 mRNA in BALB/c mouse macrophages were observed by the radioimmunoassay, ELISA and nitric acid reduction and RT-PCR in the presence or absence of TGA. (1) TGA could significantly decrease the production of PGE2and NO in mouse peritoneal macrophages. The inhibitory rate to LPS-induced PGE2production was 87% (TGA 100 mg/L, P<0.05, vs. LPS) and 62% (TGA 20 mg/L, P<0.05, vs. LPS), respectively. The inhibitory rate of NO production in mouse peritoneal macrophages was 49% (TGA 100 mg/L, P<0.05, vs. LPS) and 21% (TGA 20 mg/L, P<0.05 vs. LPS), respectively. TGA could not inhibit LPS-induced TNF-α production in mouse peritoneal macrophages. (2) TGA also inhibited the expression of COX-1 and COX-2 mRNA in RAW264.7 cells. The inhibitory rate of TGA to COX-1 mRNA was 22% (TGA 100 mg/L, P<0.05, vs. blank). The inhibitory rate of TGA to COX-2 mRNA was 55% (TGA 20 mg/L, P<0.05, vs. LPS) and 100% (TGA 100 mg/L, P<0.01 vs. LPS), respectively. The anti-inflammatory mechanisms of TGA for inhibiting the production of NO and PGE2are through inhibiting COX-2 mRNA expression without TNF-α changes.

      Abstract:To study the clinical effect of Tongxie Yaofang (痛泻要方) Granule in treating diarrhea-predominate irritable bowel syndrome (D-IBS) and its possible mechanism. A total of 120 patients were assigned to two groups using stratified block randomization, 80 in the intervention group and 40 in the control group. To the intervention group the TXYF granule was given at one package each time, twice a day; the control group was treated with Miyarisan three times a day, two tablets each time. The course of treatment was 4 weeks for both groups. The total efficacy in them was compared, and data of scoring on stool (Bristol method), abdominal pain, abdominal distension, and mental condition were collected before treatment and 2 and 4 weeks after treatment. The activation of mast cells (MCs) of six patients chosen from each group was detected as well before and after treatment. No significant difference between the two groups in terms of the total efficacy or the scores of symptoms before and after treatment was found (P>0.05). The number of activated MCs was decreased in the intervention group after treatment, showing significant difference as compared with that before treatment as well as with that in the control group after treatment (P<0.01). TXYF is an effective preparation for the treatment of D-IBS. It can quickly lessen abdominal pain and distention, improve the property of stool, and improve mental tension and depression in patients. Its mechanism of action might be through the adjustment of MCs activation to decrease visceral hypersensitivity.

      Abstract:To investigate the relationship between various Chinese medicine (CM) types and T-cell subsets (CD4+ and CD8+) in the colonic mucous membranes of patients with ulcerative colitis (UC). Fifty UC patients were enrolled, after differentiation into four types by CM syndromes, i.e., the internal heat-damp accumulation type (IHDA), the qi-stagnancy with blood stasis type (QSBS), the Pi (脾)-Shen (肾) yang-deficiency type (PSYD) and the yin-blood deficiency type (YBD). From every patient, 3–5 pieces of intestinal mucous membrane tissues were taken through colonoscopy to determine the levels of the T-cell subsets (CD4+ and CD8+) using immunohistochemical method. The results were compared with those in the normal control. The level of CD8+increased and the ratio of CD4+/CD8+decreased mainly in colonic mucous membranous tissues in UC patients. The level of CD4+decreased significantly in IHDA types (P<0.01), but decreased only slightly in the PSYD, QSBS and YBD types. CD8+increased significantly in the IHDA types (P<0.01), but only slightly in the other three types. The IHDA types of UC are closely related with T-cell subsets. The difference of T-cell subsets in various IHDA types of UC patients has provided a theoretical basis for syndrome differentiation in the CM typing of UC.

      Abstract:This paper intends to explore the color changes considered to be Maillard reaction during the process of Chinese herbal medicine. The Maillard reaction products (MRPs) are often in substantial proportions of Chinese herbal compound decoctions but their effects are often neglected. By considering the effects of MRPs in studies of effective components on Chinese herbal compounds, a new perspective is established in future researches of Chinese herbal compound decoctions.

      Abstract:If adverse effects are understood as “something that happened unexpectedly” while receiving drug treatment, there is no difference between adverse effects and complications. However, acupuncture is a process different from the use of drugs. While acupuncture procedures should not be taken for granted as safe, complications causing harm are usually, according to the authors, the result of negligence, and should be differentiated from other adverse effects. Harmful complications noted consisted of bleeding and damage to the tissues and organs under the puncture sites. Adverse effects consisted of symptomatology like fainting and other systemic disturbances difficult to explain. In a study of acupuncture complications and adverse effects in 2000 procedures conducted by experienced professional acupuncturists in a research institution, it was found that complications did not occur, and adverse events happened infrequently. Attempts were made to explain the causes of adverse effects. Now that acupuncture is being widely incorporated into medical practice, a good understanding of adverse effects and complications would be necessary and helpful.

      Abstract:There are not enough clinical data about centenarians with acute myocardial infarction (AMI) undergoing percutaneous coronary intervention (PCI). A 104-year-old woman exhibited sharp chest pain and severe dyspnea. In routine examinations, her electrocardiogram showed 1–3 mm ST-segment elevation from V1 to V4 leads, the cardiac enzymes were also elevated: creatine kinase (CK)-MB was 45.7 U/L, and cardiac specific troponin I was 40 μg/L. A two-dimensional echocardiography showed anteroseptal akinesia with severely depressed left ventricular function, 38% ejection fraction. She primarily refused to receive percutaneous coronary intervention (PCI) considering her old age, and she was given a dual anti-platelet medications (low molecular heparin and frusemide). Three days later, due to continuously deteriorating angina pectoris and dyspnea, she was treated with PCI. A diagnostic percutaneous transradial coronary angiography revealed 95% stenosis in the proximal left anterior descending artery (LAD) with 90% stenosis at the origin of diagonal one. A percutaneous coronary intervention for the LAD lesion was successfully performed, and the final angiogram showed a good coronary flow without residual stenosis. The dual anti-platelet medications had to be ceased due to the upper gastrointestinal bleeding after one week and Tongguan Capsule (通冠胶囊, Chinese medicine composed with Radix Astragali, Radix Salviae Miltiorrhiae, etc.) was administered continuously. The six-month follow-up displayed a high level quality of life for the centenarian woman with the absence of angina pectoris and dyspnea. The case reinforces the importance of PCI for very elderly patients with AMI even centenarian people and reveals the possibility that Tongguan Capsule can be used to replace dual anti-platelet medication with the reduction of bleeding complications.

      Abstract:Acute myocardial infarction (AMI) is still the leading factor causing crippling and death in cardiovascular disease. Percutaneous coronary intervention (PCI) can significantly reduce inpatient mortality and incidence of complication. But owing to the existence of restenosis, in-stent thrombosis, etc., recurrent post-PCI cardiovascular events and high repeatability of hospitalization, as well as its crippling rate and mortality, remain a serious threat to the society and the patients’ family. Therefore, the appraisal and intervention in post-PCI associated risk factors has presently become one of the foci in clinical research. To improve the near- and long-term prognosis and quality of life in post-PCI AMI patients, further improvement of the evaluation system in risk factors and prognosis is necessary in order to provide a theoretical basis for early application of intervention in high-risk patients in clinical practice. This thesis mainly dissertates some explicit and valuable factors for clinical prognosis evaluation in recent studies, involving C-reactive protein (CRP), tumor necrosis factor-α (TNF-α), brain natriuretic peptide (BNP), atrial natriuretic peptide (ANP), Chinese medicine (TCM) syndrome, their correlation with clinical state and course of AMI, and their importance in clinical prognosis.
