Abstract:目的:评价二术解毒汤颗粒 (EZJDD) 治疗脾虚湿热型中晚期乙型肝炎病毒相关性原发性肝癌 (HBV-PLC) 的疗效和安全性.方法:2021年1月到2023年6月期间招募132例中晚期乙型肝炎病毒相关性原发性肝癌患者, 根据随机数字表法将患者分配至对照组或EZJDD组, 每组66例. 对照组患者接受常规治疗3个月, 随后进行3个月随访. 除常规治疗外, EZJDD组患者口服EZJDD颗粒 (10.9克/包, 2包, 每日两次) , 疗程相同. 采用Kaplan-Meier法评估无进展生存期 (PFS) 作为主要结局. 采用Karnofsky性能状态评分 (KPS) 评估两组患者治疗前后的生活质量, 计算患者生存率. 采用尼莫地平法评价中医证候疗效. 分别测定肝功能、肿瘤指标和T淋巴细胞亚群. 记录并评估安全性指标.结果:两组完成研究患者共116例, 其中对照组57例, EZJDD组59例. EZJDD组患者的中位无进展生存期为3.53个月 (106天) , 对照组为2.33个月 (70天, P=0.005) . 对照组及EZJDD组患者的6个月生存率分别为52.63% (30/57) 和69.49% (41/59, P=0.039) . EZJDD组患者KPS评分中位数[70(63, 90)]明显高于对照组[70(60, 80)] (P=0.013) . 对照组中医综合征治疗总有效率为52.63% (30/57) , EZJDD组为77.97% (46/59) (P=0.005) . EZJDD组AFP、AFP-L3、AFU、PIVKA Ⅱ增长幅度小于对照组, 但差异无统计学意义 (P>0.05) . EZZJD组CD8+水平降低, CD3+、CD4+水平及CD4+/CD8+比值明显升高 (P<0.05) . 研究期间未观察到与治疗相关的不良反应.结论:EZJDD颗粒明显延长中晚期HBV-PLC患者的中位无进展生存期, 提高了6个月生存率 (注册号ChiCTR2200056922) .

Abstract:目的:探讨电针 (EA) 通过抑制NOD样受体热蛋白结构域相关蛋白3 (NLRP3) 炎症小体活化继而调控NLRP3/半胱氨酸天冬氨酸蛋白酶-1 (Caspase-1) /消皮素D (GSDMD) 通路介导的胃窦组织细胞焦亡改善糖尿病胃轻瘫 (DGP) 大鼠胃运动的作用机制.方法:将40只SD大鼠随机分为对照组、DGP组、EA组及MCC950组. 采用一次性大剂量腹腔注射2%链脲佐菌素溶液配合8周的高糖、高脂饲料喂养建立DGP模型. 随后行电针干预, 以疏密波电针 "足三里" "梁门" . "三阴交" 三穴15分钟, 每疗程为期5天, 治疗3个疗程. 干预结束后, 观察各组大鼠血糖、尿糖、胃排空率及小肠推进率. HE染色观察大鼠胃窦组织病理形态, TUNEL染色检测细胞DNA损伤阳性率, Western blot法检测胃窦NLRP3、凋亡相关斑点样蛋白 (ASC) 、Caspase-1前体 (Pro-caspase-1) 、Caspase-1及GSDMD蛋白表达水平, 免疫荧光染色法检测GSDMD-N端结构域活性, 并采用生化试剂盒检测乳酸脱氢酶 (LDH) 水平.结果:造模后, DGP模型大鼠表现为持续高血糖及胃肠动力下降 (P<0.05或P<0.01) . HE染色观察显示胃窦组织存在明显的病理改变, 细胞DNA损伤明显, 组织中NLRP3、ASC、Pro-caspase-1、Caspase-1及GSDMD表达水平明显升高, GSDMD-N荧光信号增强, LDH释放量增加 (P<0.01) . 电针治疗改善大鼠高血糖水平和胃肠动力, 缓解胃窦的病理损伤 (P<0.05) . 同时明显改善胃窦细胞DNA损伤情况, 下调NLRP3、ASC、Pro-caspase-1、Caspase-1及GSDMD蛋白表达, 抑制GSDMD-N活性, 减少细胞内LDH渗漏 (P<0.05或P<0.01) . 且电针组效应与MCC950组相当.结论:电针能修复DGP大鼠胃窦病理损伤, 调节胃肠动力, 其机制可能与抑制NLRP3活化继而抑制NLRP3/Caspase-1/GSDMD信号通路介导的细胞焦亡相关.
