华佗再造丸对缺血性脑卒中恢复期患者神经功能及肢体运动功能的影响:一项开放标签、随机对照试验

Abstract:目的:评价中成药华佗再造丸对缺血性脑卒中 (IS) 恢复期患者神经功能及肢体运动功能的改善的作用.方法:本研究为前瞻性、开放标签、随机对照试验. 纳入2021年5月至2023年6月中国中医科学院西苑医院神经内科收治的IS恢复期患者, 按1:1随机分为华佗再造丸组 (40例) 与对照组 (40例) . 试验组在常规治疗基础上口服华佗再造丸 (8克/次, 每日3次) , 对照组仅接受常规治疗, 疗程均为12周. 主要结局指标为治疗6周、12周后改良Ashworth量表 (MAS) 评分较基线的变化; 次要结局包括美国国立卫生研究院卒中量表 (NIHSS) 、Fugl-Meyer运动功能评定量表 (FMA) 、Barthel指数 (BI) 评分的变化, 以及12周后血脂指标. 记录所有不良事件 (AEs) 并评估肝肾功能指标.结果:共72例患者完成研究 (华佗再造丸组38例, 对照组34例) . 意向性治疗分析和符合方案集分析均显示, 治疗6周、12周后, 华佗再造丸组MAS、NIHSS、FMA及BI评分改善均显著优于对照组 (P<0.05) . 两组12周血脂指标、AEs及肝肾功能异常发生率无统计学差异 (P>0.05) .结论:华佗再造丸可缓解IS恢复期患者的肢体痉挛, 改善神经功能及预后, 但其对IS转归的长期影响仍需通过扩大样本量及延长随访时间以进一步验证. Abstract:在过去几十年中, 全球糖尿病及其并发症的患病率急剧上升, 社会经济负担增加, 人类生命健康受到严重威胁. 因此, 迫切需要对糖尿病及并发症的补充和替代疗法进行探究. 木犀草素是一种多酚黄酮类化合物, 广泛存在于一些天然资源中, 作为一种安全膳食补充剂, 其在糖尿病及糖尿病并发症治疗方面已被广泛研究与报告. 本综述证明了木犀草素在糖尿病及并发症中的治疗潜力, 阐明了其在分子水平上的作用模式, 包括抗炎、抗氧化、神经保护、改善内皮功能、改善胰岛素抵抗及β细胞功能障碍等作用, 同时它抑制二肽基肽酶-4及α-葡萄糖苷酶活性. 然而, 由于木犀草素水溶性及口服生物利用度低, 其在医学领域的应用受到限制, 因此强调木犀草素与当前先进科学技术的联合应用, 尚且需要更多的高质量的人体临床研究来阐明木犀草素对糖尿病患者的影响.
