Abstract:Objective: To investigate the clinical effect of electro-acupuncture (EA) in treating maxillofacial hemangioma (MFH).Methods: Platinum needles (electrode) were inserted into the MFH in 405 patients, the locations of which were found with the guidance of B-ultrasonography or CT, and the number of needles was decided by the size of the MFH, the distance between needles was about 1. 5cm, and the peri-lesional health tissues were protected with plastic insulating canulae. The electrodes, i.e. the cathodes and anodes, were linked to the EA apparatus for EA treatment, and the usually applied voltage was 5.0–7.0V, the current 60–80mA and the quantity about 100 coulombs/cm of MFH’s diameter. B-ultrasonography was used for monitoring during EA, and the treatment was finished when B-ultrasonography showed disappearance of blood flow in the MFH.Results: Evaluation according to WHO’s standard for therapeutic efficacy evaluation of tumor showed that, after treatment, 345 patients (85.2%) were completely remitted (CR), 39 (9.6%) partially remitted (PR), 12 (3.0%) improved pathologically, and 9 (2.2%) showing no improvement, the effective rate (CR+PR) being 94.8%.Conclusion: EA, as a creative technique for treatment of MFH, has made a breakthrough in clinical practice, showing high efficacy. It also has such superiorities as being simple, causing less injury, and enabling the patients to recover quickly, with no scar left and easily to popularize, and therefore, it provides an effective method for curing MFH.

Abstract:Objective: To explore the possibility of using traditional Chinese medicine (TCM) in treating β-thalassemia, its clinical effect and molecular mechanism of the action. Methods: According to the TCM theory of “Shen producing marrow”, the composite recipe, Yisui Shenxueling Granule (YSSXL), consisting of Chinese drugs for nourishing Shen and supplementing marrow (NS & SM) was given orally to 78 patients with β-thalassemia (49 of the severe type and 29 of moderate type), 3 times a day, 10 g each time (for children, the dose would be reduced properly), with 3 months as one therapeutic course, and no blood transfusion used in the course. The clinical therapeutic efficacy and hematologic parameters in patients were observed, and systemic gene analysis was conducted with PAGE, PCR, PCR-SSCP, RT-PCR and DNA sequences analysis and mRNA detection, in order to study the molecular mechanism from the relationships between genetic mutation and clinical efficacy, gene expression and its regulation. Results: YSSXL showed obvious therapeutic effect in treating β-thalassemia. Gene analysis revealed that it did not change the genetic mutation type, but could obviously increase hemoglobin, fetal hemoglobin (HbF), γ/(β+γ) globin ratio, γ-globin mRNA expression and GM-CSF mRNA expression in patients, as well as the GM-CSFmRMA in marrow of mice after60Co radiation. Conclusion: YSSXL has a remarkable therapeutic effect on β-thalassemia, and its possible mechanism is its action in unlocking γ-gene, increasing the γ-globin expression and enhancing HbF synthesis so as to compensate for the gene defect. This study has opened a new path for the treatment of β-thalassemia with TCM.

Abstract:Objective: To investigate the role of CT scanning in diagnosing severe acute respiratory syndrome (SARS).Methods: One hundred and twelve times of spiral CT scanning, 106 times on the chest with standard pulmonary and mediastinal window, 5 on the brain and once on the abdomen, were performed in 82 patients (37 males and 45 females) of SARS.Results: Bilateral shadows showed in 66 patients (80.48%) and unilateral shadow in 16 (19.52%). The lung CT findings were sub-pleural focal consolidation in 26 patients (31.70%), flaky cloudy opacity in 53 (64.63%), large area consolidation in 9 (10.97%), ground-glass blurry shadow in 31 (37.80%), alveolar substantive shadow in 14 (17.07%) and interstitial changes in 16 (19.51%). The pulmonary CT signs of SARS were relatively characterized by: (1) The lesions tending to multiply occur, mostly to be bilaterally distributed and commonly involved in the lower lung field. (2) The lung shadows mostly showed as sub-pleural focal consolidation, flaky cloudy shadow, large area consolidation, ground-glass blurry shadow, and often accompanied with signs of broncho-inflation. (3) Having opacified nodular shadows in the alveolar cavities. (4) Rapid progressions or changes on the size, amount, and distribution of the lesions likely to be found in dynamic observation of chest X-ray and CT scanning, i.e., markedly dynamic changes found within 24 to 48 hrs. Lesions with these characteristics may be recognized as pulmonary changes possibly induced by SARS. Complications were found in 6 patients (7.31%), including tuberculosis of lung and brain accompanied with pneumomediastinum in one patient, secondary infection of lung in 2, pneumothorax in 1, pulmonary fungus in 1, and pyothorax in 1.Conclusion: CT scanning is a sensitive method for diagnosis of SARS, by which more accurate assessment of the abnormal changes of lung and occurrence of complications in SARS patients can be made.

Abstract:Objective: To observe the clinical effect of integrative Chinese and western medicine (IC-WM) in treatment of patients with severe acute respiratory syndrome (SARS) and its influence on their T-lymphocyte subsets. Methods: Fifty-one patients with SARS of severe type were observed with synchronous non-randomized controlled method. They were divided into the ICWM group (29 patients) and the western medicine (WM) group (22 patients). Western medical treatment was applied to both groups, but to the ICWM group, Chinese medicine was given additionally. The therapeutic course was 2–3 weeks for both groups. Clinical effect and changes of T-lymphocyte subsets (CD4+) after treatment were observed. Results: In the ICWM group, 26 patients (89. 66%) were cured and 3 (10. 34%) died, while in the WM group, 12 (54. 55%) cured and 10 (45. 45%) died, thus comparison of the cure rate between the two groups showing significant difference (P <0.01). The score of clinical symptoms in the ICWM group was decreased from 7.14±5.20 scores before treatment to 1.82±3.75 scores after treatment, while in the WM group, it lowered from 7. 36±3.84 scores before treatment to517±417 scores after treatment, significant difference shown in the comparison of the values between the two groups after treatment (P < 0.01). Immunological function test showed that CD4+ T-lymphocyte in the ICWM group rose from 361 ± 278 cells/mm3 before treatment to 630 ± 454 cells/mm3 after treatment, showing significant difference (P < 001); but in the WM group, it merely rose from 288±186 cells/mm3 to 376±285 cells/mm3 in the corresponding period (P > 0.05).Conclusion: ICWM could improve the clinical symptoms of SARS patients markedly, enhance their T-lymphocyte immune function, and reduce their mortality.

Abstract:Objective: To observe the relationship of deep-slow respiratory pattern and respiratory impedance(RI) in patients with chronic obstructive pulmonary disease (COPD).Methods: RI under normal respiration and during deep-slow respiration was measured one after the other with impulse oscillometry for 8 patients with COPD and for 9 healthy volunteers as control. Results: When respiration was changed from normal pattern to the deep-slow pattern, the tidal volume increased and respiratory frequency significantly decreased in both groups (P < 0. 01), the total respiratory impedance (Z respir) showed a decreasing trend in COPD group, but with no obvious change in the control group. No change in the resonant frequency (fres) was found in both groups, and the respiratory viscous resistance obviously decreased in the COPD group(R5: P=0.0168; R20: P=0.0498; R5-R20: P=0.0388), though in the control group it was unchanged. Conclusion: IOS detection could reflect the response heterogeneity of different compartments of respiratory system during tidal breathing. During deep-slow respiration, the viscous resistance in both central airway and peripheral airway was decreased in patients with COPD. RI measurement by impulse oscillometry may be a convenient pathophysiological method for studying the application of breathing exercise in patients with COPD.
Abstract:Objective: To study the therapeutic effect of Compound Biejia Ruangan prescription (CBRP) on rat model with pulmonary fibrosis induced by bleomycin. Methods: Fifty-four male Sprague-Dawley rats were randomly divided into 6 groups (9 rats in each group). From the first day to the 28th day of the experiment, except to those in the sham-model control group that were treated with normal saline, the same amount of bleomycin injection as the normal saline given to the control group was given through endotracheal instillation to all the rats in all the other groups. From the 29th day of the modeling, CBRP solution of different dosages was respectively injected into the rats in the high, moderate and low CBRP dose group, while equal volume of normal saline was given to those in the sham-model control group and the model control group, and an equal volume of prednisone solution was given to rats in the prednisone group. On the 80th day, the high-resolution computerized tomographic (HRCT) images were observed on an equal footing, and HRCT-pathology was correlatively studied. Results: Different HRCT pathological changes were shown in the rats with pulmonary fibrosis, such as lung consolidation, thickening of interlobular septum and interlobular mesenchyma as well as lobular deformation, nodule shadow, abnormal brochiovascular tract, thickened pleura with irregular junction and polished glass-like dense shadows. Honeycomb lung was observed in some cases. Pathological sections showed fibrotic proliferation of lung tissues and noticeable pulmonary interstitial fibrosis. CBRP could improve HRCT images of rats with pulmonary fibrosis, and lower fibrotic proliferation of the lung tissue. Conclusion: CBRP plays its therapeutic role possibly through its effect on the structure of the lung in rats with pulmonary fibrosis.
Abstract:Objective: To observe the blocking effect of Huoxiang Zhengqi liquid HXZQL) on histamine release of basophils. Methods: The study was conducted using HXZQL to neutralize and block IgE and antagonize the effect of IL-3 induced enhancing histamine release of bosaphils. Concentration and percentage of histamine release of bosaphils were tested by the basophil histamine release test. Results: The HXZQL drug-serum had obvious inhibitive effect on degranulation of bosaphils induced by IgE antigen-antibody complex (P < 0. 05) and can successfully weaken the histamine release of basophils stimulated by IL-3. When compared with those in the untreated group, it showedP < 0. 01.Conclusion: HXZQL drug-serum may neutralize and block IgE and has inhibitive effects on degranulation of cells when resensitized by antigens. It can also block allergic response type I by antagonizing the effect of IL-3 induced enhancing histamine release.
Abstract:Objective: To investigate the distribution and recruitment of pulmonary dendritic cells (DCs) and the influence of low dosage arsenic trioxide (As2O3) on them in the airway of asthmatic mice. Methods: Thirty BALB/c mice were randomly divided into 3 groups: the control group, the asthmatic group and the As2 O3 treated group. The mice asthmatic model was induced via sensitizing with peritoneal injection of ovalbumin (OVA) for two times and then provocated with aerosol inhalation of OVA for a week. The treated group was peritoneally injected with 0. 2 ml solution of As2O3 (4mg/kg) 0. 5h after each provocation. The immunohistochemistry and computerised image analysis were applied to detect quantitatively the DCs in the lung and airway of mice. Results: All intraepithelial nonlymphoid dendritic cells-145 (NLDC-145) throughout the respiratory tree in the mice of the control group formed a network with the density of DCs varying from (575±54) cells/mm2 epithelial surface in the large airway, to (68±12) cells/ mm2 epithelial surface in the small airway. The distribution of airway NLDC-145+ in the asthmatic group was similar to that in the control group, but its density was significantly upregulated (P < 0.01). The distribution of airway NLDC-145 in the treated group was similar to that in the asthmatic group, only its density was significantly downregulated (P < 0.01).Conclusion: There is an integral network of NLDC-145+ throughout the respiratory tree. To downregulate the density but not change the distribution of pulmonary DCs could be an important therapeutic mechanism of low dosage As2 O3 in treating asthma.
Abstract:Objective: To investigate muscone’s neuroprotective action and its mechanism in rats with acute cerebral ischemic injury. Methods: Rat focal ischemic model of middle cerebral artery occlusion (MCAO) was established by thread inserting method. TTC staining, RT-PCR and Western blot analysis were used to study the effects of muscone on infracted volume, EAAClmRNA and protein levels. Results: Infarcted volume in the muscone treated group was significantly lower than that in the model control group. Compared with normal group, EAAC1 mRNA level increased in the model control group at 6 and 24 hrs, and protein level increased at 24 hrs after modelling, muscone could reduce the increased expression in model rats. Conclusion: Muscone possesses the action of neuroprotection, its mechanism may be related to reduction of EAAC1 and protein expressions.
