Abstract:Objective: To evaluate the therapeutic and adverse effect of small dose of aspirin (Asp) combined with Ginkgo-damole injection (GDI) in treating senile unstable angina pectoris (UA).Methods: One hundred and twenty old in-patients of coronary heart disease with UA were randomly divided into two groups. The 60 patients in the treated group were treated with oral administration of enteric soluble 50 mg Asp once a day and GDI 20 ml per day by intravenous dripping, with 14 days as one therapeutic course, while the other 60 in the control group were treated with enteric soluble Asp 100 mg alone once a day orally. Besides, isosorbide-5-mononitrate 20 mg twice a day was applied to both groups, and the β-adrenoceptor blocker, blood lipids regulatory agents and nitroglycerin (10 mg by intravenous dripping) were given accordingly. The angina total improving rate, hemorrheologic indexes (whole blood viscosity, plasma viscosity, fibrinogen, platelet aggregation rate), comprehensive clinical terminal event and the total occurrence rate of adverse reaction in the two groups were observed. Results: After treatment, comparison between the two groups showed insignificant difference in aspects of angina total improving rate (75.00% vs 65.00%), hemorrheological indexes and comprehensive clinical terminal event rate (25.00% vs 31.67%),P>0.05, and the hemorrheological indexes were improved in both groups (P<0.05), but the total occurrence rate of adverse reaction in the treated group was lower than that in the control group (6.67% vs 25.00%), showing significant difference (P<0.05).Conclusion: In treatment of senile UA, small dose of Asp combined with GDI showed therapeutic effect similar to that of conventional dose of Asp, but it has lower adverse reaction.

Abstract:Objective: To study the clinical effect of Fuzheng Jiedu granule (FJG) and its influence on serum nitric oxide (NO), nitric oxide synthase (NOS) and microelements in patients with coronary heart disease who are in long-term contact with nickel. Methods: Sixty patients were randomly divided into two groups, the treated group and the control group, 30 in each group. Conventional Western medicinal treatment was applied to both groups, but to the treated group, FJG was given additionally by oral intake, and the therapeutic course for both groups was 4 weeks. Besides, a health control group was set up with 15 healthy subjects selected from the same area, who had no history of contact with nickel. Clinical symptoms of the patients and changes in their electrocardiogram (ECG) and serum levels of NO, NOS and microelements were observed before and after treatment. Results: The total effective rate on clinical symptoms in the treated group was 93.3% and that in the control group 63.3%, comparison between the two showing significant difference (P<0.05); the total effective rate on ECG in the two groups was 90.0% and 56.7% respectively, which also showed significant difference (P<0.05). After treatment, levels of serum NO, NOS, Mg, Zn and Ca increased and those of Ni, Mn and Fe decreased significantly (P<0.05 orP<0.01) in the treated group, while in the control group, changes in these laboratory indexes were insignificant. Conclusion: FJG could lower the blood nickel level and regulate the unbalance of microelements, help restore serum NO and NOS in persons exposed to nickel and improve the clinical symptoms of patients with coronary heart disease and in long-term contact with nickel.

Abstract:Objective: To investigate the effects of Xuezhikang capsule (XZK) on blood lipids level, platelet activation and coagulation-fibrinolysis activity in patients with hyerlipidemia. Methods: Seventy-six patients of hyperlipidemia were randomly divided into two groups, the XZK group (n = 38) treated with XZK 600mg, taken two times per day and the Simvastatin (SIM) group (n = 38) treated with SIM 20mg per day, with the treatment lasting 8 weeks for both groups. Levels of fasting serum lipids, including total cholesterol (TC), triglyceride (TG), high and low density lipoprotein cholesterol (HDL-C and LDL-C), plasma GMP-140, fibrinogen (FIB), tissue plasminogen activator (t-PA), plasminogen activator inhibitor type-1 (PAI-) and prothrombin time (PT) were all measured before and 8 weeks after treatment. Data were compared before and after treatment and also compared with those measured in 20 healthy subjects of control. Results: Before treantment the levels of TC, TG and LDL-C were obviously higher and HDL-C level was significantly lower in hyperlipidemia patients than those in healthy subjects (P<0.05 orP<0.01). After 4–8 weeks of XZK treatment, the levels of TC, TG, LDL-C and FIB and activities of GMP-140 and PAI-1 were obviously lowered (P<0.05 orP<0.01). But in the SIM group there was no obvious change in FIB (P>0.05), instead it showed obvious increase of HDL-C and decrease of TC and LDL-C after treatment (P<0.05 orP<0.01).Conclusion: XZK could inhibit platelet activity and improve coagulation-fibrinolysis function, besides its lipids lowering effect.

Abstract:Objective: To explore the role of cytokines on the pathogenesis of atherosclerosis, and the effect of Tongmai Jiangzhi oral liquid (TMJZ) on cytokines through observing serum P-selectins (Ps), von Willebrand (vWF), and D-dimer (D-D) in atherosclerosis (AS) patients. Methods: Sixty-three AS patients were randomly divided into the treated group (n = 33, treated with TMJZ, 10 ml each time, three times a day) and the control group (n = 30, treated with Lovastatin, 10 mg, once daily). The levels of serum lipids (enzymatic methods), Ps, vWF, and D-D were measured before and after 8 weeks of treatment. Results: Serum total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), apo-protein B-100 (apoB-100) were significantly decreased (P<0.05 orP<0.01) and high-density lipoprotein cholesterol (HDL-C) was significantly raised (P<0.05) after TMJZ treatment. Serum Ps, vWF, and D-D also declined (P<0.05) after treatment. There was no significant difference between the treated and the control groups in these parameters except serum HDL-C level. Conclusion: TMJZ has good therapeutic effect in regulating serum lipids, improving endothelial cell function, inhibiting activation of platelets, and preventing the disturbance of blood coagulation/fibrinolysis function in patients with AS.

Abstract:Objective: To observe the effect and clinical significance of circulating endothelial cells (CEC) in the pathogenesis of coronary heart disease with unstable angina pectoris (CHD-UAP), and to explore the protective effect of Chinese herbs for activating blood circulation, removing stasis and supplementing Qi (CH) on CHD-UAP patient’s CEC.Methods: Sixty patients with diagnosis of CHD-UAP confirmed and differentiated to be Qi-deficiency and blood stasis by TCM were randomly divided into two groups and treated, on the basis of Western drug-therapy, with Tongxinluo capsule,(TXL) and Huoxue Tongmai capsule (HXTM) respectively by way of oral taking three times a day, 3 capsules every time, with 1 month as one therapeutic course. The number of CEC in patients’ blood circulation was counted before and after treatment. Besides, the number of CEC in 30 healthy persons was also counted for control. Results: The number of CEC in CHD-UAP patients was significantly higher than that in the healthy persons (P<0.01). After the patients were treated with CH, either TXL or HXTM, it significantly decreased (P<0.01) with insignificant difference between the two treated groups. Conclusion: CEC in CHD-UAP patients is severely damaged and endothelial function in disorder, Chinese herbs have protective effect on patients’ CEC.

Abstract:Objective: To observe the effect and adverse reaction of tripterygium polyglycosides (TP) combined with small dose of methotrexate (MTX) in treating senile rheumatoid arthritis (RA).Methods: Sixty old patients with RA were equally divided into two groups, the treated group was treated with oral intake of TP (10 mg three times per day) plus MTX (7.5 mg, once per week), and the control group was treated with MTX (15 mg per week) respectively for 3 months. The changes from before to after treatment in resting pain, morning rigidity, bilateral grip strength, local tenderness index, arthroncus index, patient’s evaluation, doctor’s evaluation, erythrocyte sedimentation rate (ESR), C-reaction protein (CRP) and rheumatoid factor(RF) titre were observed and the clinical therapeutic effect assessed. Results: The total effective rate in the treated group was 86.67% and that in the control group 83.33%, showing insignificant difference between them (P>0.05). But the improvement in local tenderness index, arthroncus index, morning rigidity and ESR in the treated group was significantly better than those in the control group (P<0.01 orP<0.05). The occurrence rate of adverse reaction in the treated group was 26.67%, which was lower than that in the control group (43.33%), showing significant difference (P<0.01).Conclusion: TP combined with small dose of MTX is a favorable therapeutic method for treating senile RA.

Abstract:Objective: To investigate the effects of kudzuvine root on 3 kinds of brain evoked potential in patients with diabetes mellitus type 2 (DM2).Methods: Forty-six cases of DM2 were randomized into two groups: the control group and the treatment group, they were respectively treated orally with 50 ml of normal saline or 50ml of kudzuvine root decoction containing kudzuvine root 0.4 g/ml. The short latency somatosensory evoked potential (SSEP), brainstem auditory evoked potential (BAEP) and visual evoked potential (VEP) in patients were measured before and 40 min after treatment. Results: (1) SSEP in the control group was not changed significantly before and after taking normal saline (P>0.05), while in the treatment group there appeared a significant delay in peak latency (PL) of N13 wave (N13PL) and a significant shortening in N20PL and interpeak latency (IPL) between N13 wave and N20 wave (N13-N20 IPL) after kudzuvine treatment (P<0.05 orP<0.01). But the difference of various parameters of SSEP between the two groups was insignificant difference (P>0.05). (2) BAEP in the control group was not changed significantly after treatment (P>0.05), while in the treatment group I-III IPL and I-V IPL were significantly shortened, showing significant difference as compared with those in the control group (bothP<0.01). (3) VEP in the control group was not changed significantly after treatment (P>0.05), while in the treatment group the amplitude of P100 wave increased obviously (P<0.01). But the difference of various VEP parameters between the two groups was not significant statistically after treatmentP>0.05).Conclusion: Kudzuvine could shorten the latent period of brain evoked potential and the central transmission time, increase the amplitude of cortex wave and turn it back to normal, thus to effectively protect and accommodate the brain function in patients with DM2.

Abstract:Objective: To explore the therapeutic mechanism of Bushen Yiqi Hexue recipe (BYHR), and observe its effect on pinopodes expression on endometrial surface in mifepristone induced embryo implantation dysfunctional mice. Methods: Pregnant Kunming mice were randomly divided into the normal group, the control group and the treated group. Mice in the treated group were administered with BYHR, and those in the normal group and the control group were administered with normal saline, starting from the very first day of pregnancy (Pd1). On the Pd4, mifepristone (RU486) was subcutaneously injected into mice in the control group and the treated group. Pinopodes expression on endometrial surface at 09:30–10:00 pm on Pd4 (regarded as time point 1, T1) and at 09:30–10:00 am on Pd5 (as time point 2, T2) was determined by scanning electron microscopy. Pregnancy rate and average implanted embryos were observed on Pd7.Results: Pregnancy rate and average implanted embryos in the control group were obviously lower than those in the normal group (allP<0.01), while the two parameters were significantly higher in the treated group (P<0.05,P<0.01, respectively) than those in the control group. In the normal group, abundant developing pinopodes were distributed over the whole endometrial surface in T1, and they were altered to a great deal of fully developed pinopodes in T2. But in the control group, only a few pinopodes were expressed locally on endometrial surface in T1, showing a nonsynchronous figure of development, and they completely disappeared in T2. In the treated group, a lot of developing pinopodes were expressed like those in the normal group but somewhat lagged behind, whereas many fully developed pinopodes were expressed in T2.Conclusion: The decrease of the pinopodes in Tl and the cleaning up of them in T2 is possibly the partial mechanism of mifepristone in inducing embryo implantation dysfunction. It is indicated that BYHR could improve pinopodes expression on endometrial surface, eventually better the uterine receptivity and improve the embryo implantation.

Abstract:Objective: To explore changes of neural stem cells (NSCs) following intracerebral hemorrhage (ICH) and the effect of Nao Yi-an granule (NYA) on them, and furthermore, to explore NYA’s therapeutic mechanismMethods: Sixty rats were randomly divided into three groups, the sham operation control group, the ICH group and the NYA-treated group. Except those in the sham operation control group, all other rats were replicated to ICH models by stereotactical injection of collagenase type VU into their globus pallidus. After modeling, the rats in the NYA-treated group were administered with NYA at the dosage of 4.92 g·kg-1·d-1 by gastrogavage, while those in the other two groups, 4 ml distilled water were given instead. The behavioral test was used to evaluate neurological deficit and immunohistochemical method was used to detect Nestin expression, the special marker for neural stem cells. Results: The ICH animals showed notable placing deficit in forlimbs from day 1 to day 28 after modeling. The deficit in the NYA-treated group was similar to that of the ICH group, but the former got recovered better than the latter from day 21 to 28, with significant difference shown (P<0.05). Nestin-positive cells (N+ cells) could be seen around the hemotoma from day 2 on in the ICH group, the number of which gradually increased from day 4 to 7 (P< 0.01) and reached the peak on day 14 (P<0.05), but reduced significantly on day 28 (P<0.01). Compared with that of the ICH group at the same time points, there was no significant difference in number of N+ cells in the NYA group on day 2 and 4, but it reached the peak earlier on day 7 (P<0.01), with the level significantly higher than that in the ICH group (P<0.01), and this high level lasted to day 14.Conclusion: NYA could promote the recovery of behavioral deficit in collagenase type VII-induced ICH rats, and increase the number of nestin-immunoreactive NSCs, suggesting that its effect on NSCs may be one of its pharmaceutical mechanisms in treating ICH.

Abstract:Objective: To study the effect of Danshao Huaxian capsule (DSHX), a traditional Chinese medical prescription, on the expression of collagen (Col) I, III and cysteinyl aspartate specific proteases-3 (caspase-3) in CCl4-induced hepatic fibrosis in rats. And also it is to explore the mechanism of DSHX in anti-fibrosis.Methods: Eighty male Wistar rats were randomly divided into the normal control group (A), the model group (B), the un-treated model group (C), the low-dose-DSHX treated group (D) and the high-dose-DSHX treated group (E). Except those in Group A, all the other rats were made into hepatic fibrotic models by comprehensive processes including subcutaneous injecting of CCl4, and feeding them with alcoholic high-fat and low-protein diet for 8 weeks. Then the two DSHX-treated groups were treated respectively with low dose (0.5 g/kg) and high dose (1.0 g/kg) DSHX capsule by gastrogavage everyday for 8 weeks. At the end of the experiment, the liver index, levels of hyaluronic acid (HA) and alanine aminotransferase (ALT) in serum, degree of hepatic fibrosis, and urinary excretion of hydroxyproline (Hyp) were measured, and the expression of Col I, Col III and caspase-3 in liver tissues were detected respectively by immunohistochemistric technique. Results: Compared with those in Group B and C, the two DSHX treated groups showed that the liver index, levels of serum HA and ALT and severity of hepatic fibrosis were all significantly lower, the urinary excretion of Hyp was significantly higher; the Col I and Col-III expression was lower (Col I: 1.23±1.14, 1.07 ±0.96 vs 4.18±2.26, 3.22±1.44,P<0.01;Col III: 1.31±0.69, 1.09±0.58 vs 3.04±0.62, 2.23±0.58,P<0.05). At the same time, the expression of caspase-3 in Group E was fewer than Group B and C in hepatocytes (3.09±0.65 vs 9.60±2.32, 8.82±1.45,P<0.01), but it was extensively expressed in fibrous septal cells(4.52±0.87 vs 1.69±0. 23, 2. 98±0. 36,P<0.01).Conclusion: DSHX capsule shows certain therapeutic effect on hepatic fibrosis in rats, and the mechanism might be related with reducing Col I and Col III deposition, inhibiting hepatocyte apoptosis and promoting fibrous septal cells (mainly the activated hepatic stellate cells) apoptosis.
