Abstract:Objective: To understand the clinical manifestation of severe acute respiratory syndrome (SARS) and explore its effective treatment with integrative Chinese and western medicine (ICWM).Methods: The data of patients, whose diagnosis of SARS had been confirmed were summarized and analyzed, and clinical observation was conducted when the patients were treated with ICWM.Results: In the early stage of the 42 patients, the symptoms revealed were fever in 100% of SARS patients, headache in 92. 9%, aversion to cold in 76. 2%, chest stuffiness in 76. 2%, cough in 73. 8% and myalgia in 88. 1%; pulmonary lesion involves ≥3 lobes in 42. 9%, 2 lobes in 47. 6% and 1 lobe in 9. 5%; 61. 9% of them showed liver function abnormality (increase of ALT or AST), 47. 6% showed elevated myocardial enzyme (CK or CK-MB), 0. 48% showed an inclination of renal function (higher of BUN or Cr); in their T lymphocyte subsets, 91. 2% (31/34 patients) had lowered CD3 and 76. 5% (26/34 patients) lowered CD4/CD8 ratio. In the mid-late stage, the symptoms were lassitude and weakness in 85. 7%, scare in 81. 0%, short of breath or chest stuffiness in 71. 4%, loss of appetite in 64. 3%; light dark tongue proper in 52.4%, yellow and white tongue coating in 45. 2%, and yellow thick coating on the middle-root part of the tongue in 21. 4%. Most of them were asymptomatic when discharged from hospital, with 92. 8% of their pulmonary lesion, according to chest film, completely absorbed and liver function, myocardial enzyme and renal function all normalized. However, of the 30 patients who had CD3 reexamination, 70% of the CD3 showed lower than normal range and 36. 7% showed their CD4/CD8 inclined to lower margin, follow-up should be done for these patients. Of the 42 patients, who received western medicine (WM) alone in the early stage and ICWM in the mid-late stage, 10 were severe cases and 3 critical cases, but none of them died. The mean defervescent time was 3. 52±0. 85 days, the time for complete absorption of pulmonary lesion judged by chest X-ray film was 26. 82±5. 98 days, and the mean hospitalization time was 33. 60±4. 37 days. Conclusion: The manifestation of SARS is multifarious, showing that there were damage in multiple organs. The T lymphocyte count percentage and its subsets, CD3 and CD4 /CD8 ratio, are valuable for early diagnosis and follow-up in the rehabilitation stage. Majority of the patients could be clinically cured. Combined treatment of WM and TCM according to syndrome differentiation and psychiatric intervention are beneficial to remit partial symptoms and promote rehabilitation.

Abstract:Objective: To evaluate the efficacy of integrative Chinese and western medicine (ICWM) in treating severe acute respiratory syndrome (SARS) patients. Methods: Through parallel control design, 49 SARS patients were observed. Used as control, there were in the western medicine (WM) group 29 patients, who were treated with Ribavirin, Levofloxacin, Thymopentin, Azithromycin, methylprednisolone, etc., on the basis of “Recommended Protocol for Infectious Atypical Pneumonia” (abbreviated as “Recommended Protocol”) issueed by Ministry of Health. As the treated group, there were in the ICWM 20 cases. The protocol for treatment of SARS in “Special Science and Technological Action to Prevent and Treat SARS” (abbreviated as “Special S-T Action”), issued by Ministry of Science and Technology, together with the same WM as those for the control group. Results: (1) Time from the disease onset to the symptom improvement were 5.10±2. 83 days and 7. 62±2. 27 days in ICWM and WM group respectively,P < 0.05; (2) As to corticosteroid (CS) amount and days before reducing dosage, 2 groups showed no significant difference,P > 0.05; (3) There was no significant difference in the time from disease onset to the body temperature normalization and the total amount of CS and the duration of using CS before reducing it to 80 mg between the ICWM group and the WM group; (4) The days and amounts for use CS after reducing between the ICWM group and the WM group were significantly different (P < 0. 05).Conclusion: There were obvious advantages in ICWM to treat SARS, compared with that of WM alone, especially in improving the clinical symptoms, promoting the recovery of immune function, promoting the absorption of pulmonary inflammation and reducing the dosage and duration of CS treatment.

Abstract:Objective: To evaluate the effective treatment with integrative Chinese and western medicine (ICWM) of severe acute respiratory syndrome (SARS).Methods: The 65 cases of SARS patients were treated with ICWM and 37 cases with western medicine (WM) alone as the control in our hospital from March 11 to April 30, 2003. The results were analyzed, with T subset profile tested for all the 102 patients. Results: Sixty-five cases of SARS patients treated with ICWM included 52 mild cases of whom 51 cases were cured (98.1%) and no patient died. As to the 13 severe cases in this group, 9 were cured (69. 2%) and 2 died (15. 4%). Of the 37 cases treated with WM alone, 16 out of the 18 common cases were cured (88. 9%) and 10 out of the 19 severe cases were cured (52. 6%), with the other 9 died (47. 4%). Comparison between the two groups (P=0. 061) showed the difference was close to the value for statistical significance. But considering that the number of samples was small, more cases are needed for further study. The immune function examination: in cases of the common type, after ICWM treatment, CD4 + T lymphocytes were increased from 360±247 cell/mm3 to 563±479 cell/mm3 (P < 0.05). In the group treated with WM alone, CD4 + T lymphocytes were increased from 331±193 cell/mm3 to 772±354 cell/mm3 (P < 0.05). As to the severe cases after ICWM treatment, CD4 + T lymphocytes increased from 352±279 cell/mm3 to 525±490 cell/mm3 (P > 0.05). In the group of severe cases treated with WM alone, CD4 + T lymphocytes reduced from 229±69 cell/mm3 to 205±108 cell/mm3 (P > 0.05). While after ICWM treatment, CD4 + T lymphocytes were significantly higher than that of the group treated with WM alone (P < 0.05).Conclusion: Compared with the group treated with WM alone, ICWM can significantly improve the prognosis, reduce the mortality as well as improve the immune function of SARS patients.

Abstract:Objective: To explore the effect of Gushen tablet (GST) in reducing the relapse of children’s nephrotic syndrome and the possible mechanism of drugs used. Methods: Fifty children with primary nephrotic syndrome who had been induced and alleviated with regular glucocorticoid (GC) were randomly divided into two groups: the GST group used GST and standard middle-long term course of GC, and the control group adopted standard middle-long term course of GC and immunoinhibitory or immunomodulatory agents for treatment. The 0. 5, 1 and 2 years after the treatment the relapse episodes, time for urinary protein negative conversion after relapse, the episodes of patients infection and relapse after infection were evaluated. Before and after treatment the plasma cortisol and T lymphocyte subpopulation were determined. Results: The relapse rate of GST group: the rates after 0. 5, 1, 2 years were 20. 0%, 30. 0% and 40. 9%, and the frequent relapse rate were 0, 6. 7% and 9. 2% respectively, which were lower than those of control group (60. 0%, 70. 0%, 69. 2% and 25. 0%, 15. 0%, 15. 4% respectively); in the GST group no relapse occurred within 0. 5 year, the relapse rate after 1 and 2 years reduced by 40. 0% and 28. 3%, compared with those of the control group (allP < 0.05); during the observation period, the mean infection/every child patient was 1. 86 episodes in GST group, after infection the nephrotic relapse rate was 28. 3%, which was lower than that of the control group (2. 25 episodes, 71.1%,P < 0.05); the relapse per patient in GST group was 0. 8 episodes, time for urinary protein negative conversion was 12. 00± 8. 98 days, lower than those of control group (1. 6 episodes, 20. 75±11. 95 days,P < 0. 05); 3 months after GST treatment the plasma cortisol level normalized, and the CD4/CD8 ratio elevated (P < 0.05).Conclusion: GST could possibly reduce the relapse of children nephrosis, and the frequent relapse and relapse episodes, and the time for post-relaptic urinary protein negative conversion shortened, the plasma cortisol elevated, and the adjustment of cellular immunity disturbance promoted.

Abstract:Objective: To observe the relationship of tumor necrosis factor-α (TNF-α) and nitrogen oxide (NO) with the treatment of frequent relapse nephrotic syndrome (FRNS) and to explore the pathogenesis of FRNS and the therapeutic mechanism of Shenkangling (SKL) Granule in children. Methods: Sixty children suffering from FRNS were randomly divided into the treated group and control group, 30 in each, and the other 30 healthy children were taken as healthy group. The patients were treated with prednisone for a long-term course, and those with no effect or partial effect shown were treated with additional Tripterygium or Cytoxan in the control group, while in the treated group patients were treated with prednisone and additional SKL. The two groups were compared as to their changes of TNF-α, NO before and after treatment, and the relapses after treatment. Results: The levels of TNF-α and NO in the sick children before treatment were markedly higher than those after treatment and normal group (P < 0.01). The positive correlation between TNF-α of FRNS cases and relapse risk displayed more significance than that between the relapse of FRNS and NO. The difference between treated group and control group was significant (P < 0.01).Conclusion: TNF-α can be regarded as the monitoring parameter of the active phase in FRNS, and the higher the level, the more possible the relapse would occur. SKL could markedly reduce the relapse rate of FRNS in children.

Abstract:Objective: To evaluate the clinical efficacy of Qianlie’an (QLA) suppository via anal route administration in treating chronic prostatitis syndrome. Methods: A randomized open-labelled prospective controlled trial was carried out. The total of 120 patients with chronic prostatitis syndrome were randomly divided into 2 groups: 60 patients in the treated group who were treated with QLA suppository combined with ofloxacin, and the other 60 patients in the control group who were given ofloxacin alone. The efficacy was evaluated by WBC count in the expressed prostatic secretion (EPS) and the Chronic Prostatitis Symptom Index (CPSI) made by the National Institute of Health (NIH). The clinical effects were also observed in a 4-week follow-up.Results: All but six cases completed the trial and the follow-up. It showed that in the treated group recovery rate was 17. 2%, markedly effective rate 34. 5%, effective rate 32. 8%, total markedly effective rate 51. 7%, and total effective rate 84. 5%, all of which were superior to those in the control group (total markedly effective rate 32.1% and total effective rate 66.1%, respectively),P < 0.01.Conclusion: Administration of QLA suppository via anal route combined with oral antibiotics is an effective therapy for chronic prostatitis syndrome. It can relieve the symptoms of chronic prostatitis syndrome markedly and rapidly. It is a new choice for treatment of the disease.

Abstract:Objective: To observe the effect of different dosages of ligustrazine (LG) on the level of plasminogen activator inhibitor-1 (PAI-1) activity in patients with type 2 diabetes mellitus. Methods: Ninety cases of type 2 diabetes mellitus inpatients were selected, and randomly divided into LG small dosage group (SDG), LG large dosage group (LDG) and control group. The 120 mg LG, 400 mg LG and normal saline 250 ml were given through intravenous dripping respectively, once daily, 20 days as one treatment course. Before and after treatment, all the patients had their fasting blood taken for PAI-1 and tissue plasminogen activator (t-PA) assessment test to perform the comparative study. Results: Seventythree out of the 90 patients completed the observation course, the PAI-1 activity of three groups after treatment all lowered compared with that before treatment, and the difference between groups was also significant (allP < 0.01). After treatment the PAI-1 level of SDG and LDG of LG were all markedly lowered (allP < 0.01), the LDG’s lowering was more evident than that of SDG, and comparison between these two groups of patients showed significant difference (P < 0.01). Although in the control group there was some difference between before and after treatment, it was not so significant like the above-mentioned two groups (P = 0. 0140). No adverse reaction occurred in the 3 groups during the observation period. Conclusion: LG could safely and effectively lower type 2 diabetes mellitus patient’s plasma PAI-1 activity level, and LDG of LG proved to be particularly effective.

Abstract:Objective; To explore the effects of acupuncture on the expression of uncoupling protein 1 (UCP1) gene of brown adipose tissue (BAT) in obese rats. Methods: The expression of UCP1 gene of BAT was determined with RT-PCR technique. The changes of body weight, Lee’s index, body fat, and the expression of UCP1 gene of BAT in obese rats were observed before and after acupuncture. Results: The body weight, Lee’s index, body fat in obese rats were all markedly higher than those in normal rats, but the expression of UCP1 gene of BAT in obese rats was all lower than that in normal rats. There were negative correlation between the obesity index and the expression of UCP1 gene in BAT. After acupuncture the marked effect of weight loss was achieved while the expression of UCP1 gene of BAT obviously increased in obese rats. Conclusion: The abnormal reduction for expression of UCP1 gene of BAT might be an important cause for the obesity. To promote the expression of UCP1 in obese organism might be an important cellular and molecular mechanism in anti-obesity effect by acupuncture.

Abstract:Objective: To examine the therapeutic mechanism of Yuxingeng liquid (YXGL) on early ventricular remodeling in Wistar rats with acute myocardial infarction(AMI).Methods: Measuring the cardiac index (CI), left ventricular weight (LVW) and cardiac myocyte dimension and observing the concentration of endothelin (ET) and angiotensin II (Ang II) in the plasma and myocardium. AMI models were established by ligature of left anterior descending coronary artery and the rats with AMI were randomly divided into 6 groups: the model group, sham-operation group, captopril group, high dosage YXGL group, middle dosage YXGL group and low dosage YXGL group. From the next day after modeling, the rats had been given YXGL through the gastric tube, which lasted for 4 weeks. And then, CI, LVW and concentration of ET and Ang II in the plasma and myocardium were tested. Results: Comparing with model control group, high dosage YXGL, middle dosage YXGL and captopril can all significantly reduce CI, LVW, cardiac cell dimension and content of ET and Ang II in both plasma and myocardium (P < 0.05 orP < 0.01).Conclusion: YXGL can remarkably reduce LVW, CI and concentration of ET and Ang II, and lowering the concentration of ET and Ang II is possibly one of the mechanisms intervening the pathological course of the early ventricular remodeling in rats with AMI.

Abstract:Objective: To observe the changes of coagulation and fibrinolysis function in human umbilical cord vein endothelial cells (HUVEC) induced by recombinant human tumor necrosis factor-α (rhTNF-α) and the effect of Naotaifang extract (NTE) on it.Methods: Cultured HUVEC is randomly divided into six groups: Control group, NTE control group (only 2 g/L NTE), rhTNF-α group (100 μg/ L rhTNF-α), and low-dosage, middle-dosage and high-dosage NTE group (100 μg/L rhTNF-α and 0. 67 g/L, 2 g/L, 6 g/L NTE). The coagulation activity of frozen-dissolved HUVEC, von Willebrand factor (vWF) content in the conditioned medium and tissue-type plasminogen activator (tPA) and plasminogen activator inhibitor (PAD activity were to be detected after 24 hrs. Results: Compared with the control group, PAI activity were enhanced, vWF release markedly increased in conditioned medium of TNF-α group (P < 0.01) and the frozen-dissolved HUVEC markedly shortens the rabbit plasma prothrombin time, and the above changes could be significantly inhibited by the 3 dosages of NTE (P < 0.05,P < 0. 01).Conclusion: NTE is effective in inhibiting the coagulation activity of the HUVEC non-stimulated or stimulated by rhTNF-α to enhance the vWF release, and to adjust fibrinolytic function, and mainly to inhibit the PAI activity.

Abstract:In conclusion, from the evidence gathered, it may be appropriate to suggest that CHMs may have efficacy for the treatment of patients with LC, and combination of CHMs with chemotherapy or radiotherapy may be additive or synergistic in controlling LC. In addition, CHMs may reduce some adverse effects associated with chemotherapy or radiotherapy. Moreover, CHMs may be beneficial for improving quality of life and prolonging survival time of patients with LC. However, we are cautious because of methodological flaws in the majority of the clinical studies reviewed. It is difficult to draw conclusions with regard to efficacy of CHMs. The mechanisms of actions of CHMs for LC largely remain unknown. Thus, more rigorous studies that address weaknesses identified are required for further evaluation of efficacy and safety of different Chinese herbs and formulae in the management of LC.
