Abstract:Objective: To evaluate the clinical efficacy of Shengmai Powder (SMP) in treating acute viral myocarditis objectively. Methods: One hundred and twenty-four patients with acute viral myocarditis were randomized into the treated group (SMG,n = 64) and the control group (CG,n = 60). Such myocardial nutrient medicine as ATP, CoA, Vit-C, were given to both groups. And to the treated group, 40 ml of Shengmai Injection per day was given intravenously for 2 weeks, which was followed by oral intake of Shengmai granule, one package three times daily for another 2 weeks in total. The same anti-arrhythmia agents were applied to both groups, and no fructose-1, 6-diphosphate (FDP) for either. Semi-quantitative scoring method was adopted to observe such symptoms as chest stuffiness, palpitation and chest pain before treatment and four weeks after treatment. Meanwhile, ECG, dynamic ECG by Holter monitor, left ventricular enddiastolic dimension (LVEDD), left ventricular ejection fraction (LVEF), serum neutralizing antibody of virus Coxsackie B, cardiac troponin I (cTnl) and cardiac troponin T (cTnT) were examined. Results: (1) Compared with the control group, more significant improvement was got in SMG in respects of chest stuffiness, palpitation, chest pain and arrhythmia (P<0.05 orP<0.01). (2) Negative converting rates of cTnl,cTnT in the two groups were 59.46% vs 35.48%, 68.75% vs 42.31% respectively (P<0.05). (3) LVEDD before and after treatment in SMG was 52. 44 ± 3. 40 mm and 48. 81 ± 2. 23mm respectively, while that in the control group was 52.31 ± 3. 74 mm and 49. 92 ± 2. 67mm respectively; LVEF before and after treatment in SMG was 60.67 ± 4.62% and 65. 02 ± 4.16% respectively, while that in the control group was 60. 91± 4.26% and 63.67 ± 3.17%. There was obvious improvement in the two parameters in both groups, but the improvement in SMG was superior to that in the control group (P<0.05).Conclusion: SMP shows a good effect in improving clinical symptoms and signs, heart function, abnormal ECG and inflammatory injury indexes in patients with acute viral myocarditis.

Abstract:Objective: To explore the relationship between TCM Syndrome typing and adrenocortical function in elderly patients with severe sepsis, and to see whether TCM Syndrome Differentiation can provide clinical clues in identifying relative adrenal insufficiency (RAI) in patients with severe sepsis. Methods: Sixty-one old patients with severe sepsis were classified into four types according to TCM Syndrome Differentiation: The severe invasion of toxic-heat type (Type SITH,n = 21); the Qi stagnation and blood stasis type (Type QSBS,n = 11); the sudden depletion of Yang-Qi type (Type SDYQ,n = 16); and the exhaustion of Qi-Yin type (Type EOQY,n = 13). The base-line level of plasma cortisol in patients of different types and their response to corticotropin stimulation were compared, which were also compared with those of 12 healthy elderly persons synchronously. Results: The base-line level of plasma cortisol was not significantly different between patients of different Syndrome types (P>0.05), but they were all sgnificantly higher than that in the healthy persons (P<0.05). Compared with Type QSBS and Type EOQY, Type SITH and Type SDYQ showed less cortisol concentration increment after corticotropin stimulation (P<0.05). RAI was more prevalent in patients of Type SITH and Type SDYQ than in patients of Type QSBS and Type EOQY (57% vs 25%,P<0.01).Conclusion: In old patients with severe sepsis, different TCM Syndrome types are associated with different adrenocortical function status. TCM Syndrome differentiation can provide clinical clues in identifying old patients with severe sepsis who have also RAI.

Abstract:Objective: To evaluate the efficacy and safety of Shugan Yiyang Capsule (SGYY) in treating psychogenic and mild arteriogenic erectile dysfunction (ED) of Gan stagnation and Shen deficiency Syndrome type with or without blood stasis. Methods: A multi-centered, randomized, double-blinded, placebo-controlled trial in 304 patients was conducted by dividing them into 3 groups, who received respectively SGYY (n = 102), placebo (n = 101) and Suoyang Bushen capsule (SYBS,n = 101), 1. Og taken three times a day for 4 weeks. Besides, 205 patients were arranged in an open-label study and treated with SGYY l.0 g taken three times a day for 4 weeks. The total effective rate, total remarkably effective rate, erection recovery rate tested by penis hardness test ring, time taken for improving erection among groups, as well as therapeutic effect between psychogenic ED group and mild arteriogenic ED group were compared. Adverse reactions related to SGYY were recorded. And a 3-month follow-up study was conducted on 105 cured patients. Results: The trial was completed in 500 patients. The total effective rate and total remarkably effective rate in the SGYY treated double-blinded group were 88.0% and 64.0% respectively, and those in the open-label group 90.5% and 65.0%, respectively, all significantly higher than those in the placebo (21.0%, 6.0%) and SYBS groups (60.0%, 29.0%),P<0.01. At the same time, the erection recovery rate and improving time in SGYY group were significantly better than those in other groups (P<0.01). No adverse reactions related to SGYY were found. The successful rate of potency to have intercourse within 3 months was over 80% (100 patients out of the 105 cured patients). Conclusion: SGYY was an effective and safe treatment for patients with psychogenic and mild arteriogenic ED of Gan stagnation and Shen deficiency Syndrome type with or without blood stasis, especially for psychogenic ED patients.

Abstract:Objective: To observe and evaluate the therapeutic effect of anti-tuberculosis (anti-TB) chemicals and Compound Astragalus Capsule (CAC) in combinedly treating drug resistant pulmonary tuberculosis (DR-TB).Methods: Ninety-two patients with DR-TB were equally randomized into the treated group (treated with combination therapy) and the control group (treated with anti-TB chemicals alone). The therapeutic course for both groups was 18 months. Therapeutic effects between the two groups were compared at the end of the therapeutic course. Sputum bacterial negative rate, focal absorption effective rate, cavity closing rate, 10-day symptom improving rate, the incidence of adverse reaction and 2-year bacteriological recurrence rate between the two groups were compared. Results: In the treated group, the sputum bacterial negative conversion rate was 84.8%, focal absorption effective rate 91.3%, cavity closing rate 58.7% and 10-day symptom improving rate 54.4%, while in the control group, the corresponding rates were 65.2%, 73.9%, 37.0% and 26.1%, respectively. Comparison between the groups showed significant difference in all the parameters (P<0.05,P<0.05,P<0.05 andP<0.01). The incidence of adverse reaction and 2-year bacteriological recurrence rate in the treated group were 23.9% and 2.6% respectively, while those in the control group 50.0% and 16.7%, which were higher than the former group with significant difference (P<0.01 andP<0.05, respectively). Conclusion: The therapeutic effect of combined treatment with anti-TB and CAC is superior to that of treatment with anti-TB chemicals alone, and the Chinese herbal medicine showed an adverse reaction alleviating effect, which provides a new therapy for DR-TB, and therefore, it is worth spreading in clinical practice.

Abstract:Objective: To investigate the influence of Tongxinluo capsule (TXL) on regional cerebral blood flow (rCBF) with 99mTc-ECD single photon emission computed tomography (SPECT) brain perfusion imaging, and to observe the therapeutic effects of TXL on acute small cerebral infarction (ASCI).Methods: Thirty-four patients with ASCI were enrolled and randomly divided into two groups: the control group (n = 17) was treated with the conventional treatment, i.e. 1. 0g of Citicoline added into 300 ml normal saline for intravenous dripping daily for 2 weeks and 0.8 g of Piracetam taken three times a day orally for 4 weeks, and the treatment group (n = 17) was treated additionally with 4 TXL capsules three times a day for 4 weeks besides the conventional treatment. The 99mTc-ECD SPECT brain perfusion imaging was performed before and after treatment to observe the change of rCBF, and the neurological deficit was evaluated by Edinburgh-Scandinavia stroke scale (SSS) scores and Barthel index (BI) at the same time. Results: After treatment, the rCBF in the treatment group was significantly improved (P<0.01), while that in the control group remained unchanged, with the comparison of the rCBF in the two groups after treatment showing significant difference (P<0.01). In addition, the SSS score was significantly lower and Bl significantly higher in the treatment group than those in the control group respectively after treatment. Conclusion: TXL could effectively improve rCBF and lessen the neurological deficit symptoms in patients with ASCI.

Abstract:Objective: To explore the changes of tongue pictures in the developing process of disease in patients with severe acute respiratory syndrome (SARS), and to compare the tongue pictures of SARS patients with those of suspected SARS patients. Methods: Materials of tongue picture were dynamically collected from 34 SARS patients and 35 suspected SARS patients, and the difference and similarity between them were analyzed. Meanwhile, the changes in clinic symptoms and tongue picture after integrative medicinal treatment were observed. Results: Characteristics of tongue picture in SARS patients were changed along with the progress of the disease, showing that there existed a tendency of the pathogen invading from exterior to interior, from surface to the deep. Also the tongue pictures were varied due to the coexistence of dampness pathogen; the time of the early stage was generally 1–1.5 days, shorter than that of other stages. While in the patients with suspected SARS, the tongue picture was mostly red with thin white or yellow coating, comparatively steady and showing no obvious change along with the development of the disease. Conclusion: Observing the tongue picture and its changes in different stages can be helpful to understanding the property of pathogenic evil and its developing rules, thus offering a basis for clinical treatment.

Abstract:Objective: To evaluate the effects of treatment of integrative Chinese and Western medicine (ICWM) on arterial oxygen saturation (SaO2) in patients with severe acute respiratory syndrome (SARS).Methods:The non-randomized, controlled trial was conducted on 447 SARS patients treated synchronously with western conventional treatment (WM group,n = 171) alone and ICWM (ICWM group,n = 276). The changes of the cases with normal level (≥95%) or abnormal level (<95%) SaO2 were observed dynamically. Results: In the 3rd-14th day of the therapeutic course, the percentage of patients with normal SaO2 in the ICWM group was higher than that in the WM group (OR = 0. 5178,P = 0.0038), and this tendency was more evident in patients of the severe type (OR = 0.18,P = 0.0001). However, the statistical significance of difference was only shown in patients for whom the ICWM treatment started in the early period after the onset (<-7 days after it, OR = 0.3803,P = 0.006), but not shown in those who received ICWM treatment later in the mid-late period of SARS (P>0.05).Conclusion: ICWM treatment, particularly when it is used for intervention in the early stage, is beneficial for maintaining normal SaO2 in SARS patients.

Abstract:Objective: To investigate the effect of Malytea Scurfpea fruit (MSF) on adhesion and migration of melanocyte. Methods: Human epidermal melanocytes were treated with MSF and examined for its adhesion to bovine serum fibronectin-coated culture plate and for its migration into micropore filters, which were then compared with those of untreated control. Results: Compared with the control, MSF treated melanocytes were easier to adhere to the dishes and move into the filters in a dose-dependent manner, but when the dose of MSF was higher than 150 ώ/ml, the adhesion and migration could not be significantly increased. And it was noticed that when the concentrations of MSF was l0g/ml, significant statistical difference did show between the migration of the treated and untreated melanocytes (P<0.05) though the adhesion was not increased significantly. Conclusion: MSF had the ability to affect melanocyte adhesion and migration. This could explain, in part, the mechanism of MSF in curing vitiligo lesions.

Abstract:Objective: To study the effect of Suiqing Pill (SQP), a TCM compound drug that can activate blood circulation and get rid of blood stasis, on angiogenesis in tumor and its possible mechanism.Method: BALB/c nude mice were inoculated with melanoma cell line B16 at right armpit subcutaneously to establish cancer spontaneous metastasis model. Levels of microvessel density (MVD), vascular endothelial growth factor (VEGF) and protein expression of matrix metalloproteinase-2 (MMP-2) in tumor tissues were observed and compared. Results: Strong expression of anti-Factor VIII (FVIII) related antigen and plentiful tumor angiogenesis were seen in model control animals, while in the high-dose and low-dose SQP treated model mice, MVD was inhibited by 33.5% and 22.6% respectively (P<0.01,P<0.05). The strong positive protein expression of VEGF and MMP-2 in model control also reduced in the SQP treated groups. Conclusion: SQP could inhibit tumor angiogenesis, protein expression of VEGF and MMP-2 in nude mice B16 melanoma models.

Abstract:Objective: To investigate the effects of Lirukang oral liquid (LRK) on release of neurotransmitter in rats with hyperplasia of mammary glands (HMG) and to explore its mechanism. Methods: Sixty rats were divided into six groups, the normal control group, the model control group, the large dosage (3.6g/kg) and the small dosage (1.8g/kg) LRK groups, the Ruzengning (RZN, 2.5g/kg) group and the tamoxifen (TAM, 5mg/kg) group, 10 in each group. Except those in the normal control group, all the animals were made into rat model of HMG by intraperitoneal injection of estradiol benzoate. Levels of dopamine (DA) and 5-hydroxytryptamine (5-HT) in hypothalamus and mammary gland in rats were detected by fluorescence luminosity assay, and level of prolactin (PRL) in serum was detected by radioimmunoassay. Results: In the model group, the level of DA reduced significantly (P<0.01), and 5-HT and PRL increased obviously (P<0.01). Compared with the model group, the LRK groups of both dosages and the TAM group had their level of DA significantly increased (P<0.01), and level of 5-HT significantly decreased (P<0.01). The serum PRL in both LRK groups was significantly decreased (P<0.01). No obvious changes in DA, 5-HT and PRL were found in the RZN group. Conclusion: LRK and TAM have similar effects in regulating the release of neurotransmitter in hypothalamus and mammary gland and serum content of estrogen in the animal models of HMG.

Abstract:Objective: To observe the effect of electroacupuncture (EA) on expression of p53 protein in cerebral cortex of senile rats with global cerebral ischemia/reperfusion (IR) injury and to explore its mechanism. Methods: The cerebral IR injury rat model was established referring to Pulsinelli 4-vessel occlusion method. Thirty-six SD rats were randomly and evenly divided into the control group, the IR group and the IR plus EA (IR-EA) group. The animals in the control group were subjected to electrocauterization of vertebral arteries in bilateral flank orifice alone with the general carotid arteries unoccluded. To rats in the IR-EA group, immediately and 24h, 48h, 72h after cerebral IR, EA treatment on bilateral acupoint “Zusanli” (ST36) was applied once a day, lasting for 60 minutes. After the final treatment, all the rats were sacrificed and their brains were taken to examine p53 protein expression by the immunohistochemical method. Results: Cells with positive p53 immunoreactivity in the cerebral cortex of rats in the IR group was significantly higher than that in the control group (P<0.05), while that in the IR-EA group was significantly lower than that in the IR group (P<0.05).Conclusion: EA could remarkably reduce expression of p53 protein in the cerebral cortex of senile rats with global cerebral IR injury, which might be one of the means for EA to inhibit neuronal apoptosis after cerebral IR injury.

Abstract:Objective: To study the effects of basil polysaccharide (BP) in inhibiting tumor growth and metastasis in vivo. Methods: One hundred and fifty mice were randomly divided into five groups to observe the effect on tumor growth after H22 cancer cells had been transplanted subcutaneously into their right armpit region and treated with different dosages (5 mg/kg, 2.5 mg/kg and 1.25 mg/kg) of BP for 14 days, with Mitomycin (Mit-C) used as control. Another 150 mice were randomly divided into three groups, models of tumor metastasis in the lung by various paths (lymphatic, blood circulatory and spontaneous) were established respectively. They were treated with BP or Mit-C to observe the influence of treatments on tumor metastasis by various paths. Results: BP of various dosages showed no effect on tumor growth, but in high and middle dosage, it could significantly reduce the number or metastasis nodules (P<0.05). Conclubsion: BP has a tumor metastasis inhibitory effect, which might be one of the candidates for new anti-tumor metastasis agents. Its mechanism may be blocking the function of platelets in the tumor metastasis progress.
